Freckles (ephelides) are multiple scattered brown macules/ pigmented spots observed on the face, arms, and back, easily causing cosmetic and even psychosocial concerns. Their occurrence is related to heredity and sun exposure. The lesions become darker in summer and lighter in winter.

Treatment

Other than photo protection, application of sunscreens and topicals are prescribed by the dermatologists. Picosecond laser in multiple sessions have been used to manage and control the condition. This laser removes pigment particles by its photochemical and minimal photothermal effects ( destruction of melanin due to  absorption of the wavelength).Pre treatment- sun protection, stopping of any actives 2 days prior to the treatment.

Treatment- marking of the area and  application of topical numbing cream ( local anaesthesia) for 45 min to 1 hour prior to the procedure. Parameters according to skin type is decided by the dermatologist and the laser procedure is done over the targeted area.

Post treatment- 

Sun protection is advised.Washing of face 6-8 hours post procedure with mild cleanser is advised. Avoidance of actives for 4 days post procedure and avoidance of swimming, sauna, massages for 2 weeks.

Understanding Melasma

Melasma is one of the most common pigmentary disorders worldwide with a still unresolved pathogenesis and the treatment continues to be challenging. In this patches of pigmentation are commonly seen over forehead, cheeks and nose.

Treatment available

Photo protection, sunscreen is a must for these individuals. Topical depigmenting agents (arbutin, azelaic, kojic, glycolic) are prescribed by the dermatologists along with oral medications like tranexamic acid after patient assessment.Chemical peels in multiple sessions ( glycolic , lactic acid, vitamin c) based on the extent of the lesion is advised.Picosecond laser and laser toning in multiple sessions is being done after assessing the patients.
